SUMMARY AGREEMENT FOR ATTENDING LOVEBUG & ME CLASSES...

In consideration for being permitted to participate in LoveBug & Me Music or other activities conducted by LoveBug & Me, LLC teachers, I, for myself and my children who are participants, and our respective personal representatives, heirs, executors, next of kin, and assignees, hereby acknowledge and agree that:

~ I release, waive, and discharge LoveBug & Me, LLC its owners, predecessors, successors, assignees and assignors, officers, directors, employees, volunteers, agents, venue owners and managers, and other representatives (collectively, "LB") from all liability, claims, demands, or causes of action for any and all loss, damage, or injury to person or property resulting from or relating in any manner whatsoever to any negligence, act, or omission by LB.

~ I assume full responsibility for the care and safety of my child or children who are participating or attending LoveBug & Me, LLC and understand that it is my sole responsibility to monitor their activities. I acknowledge and agree that the risks involved with taking classes with other children are acceptable to me and I agree to use my best judgment in undertaking these activities with my child or children and to follow all safety instructions.

~ I assume responsibility and agree to follow all laws and precautions regarding COVID-19, as stated by local and federal laws and our LoveBug & Me recommendations.  

I agree to the following class policies...

MAKE-UP CLASSES: If you need to miss any regular class OR make-up class, please notify us at least 2 hours prior to your missed class or the class will be forfeited. Your cooperation in this is greatly appreciated and will help ensure that everyone can make up missed classes. You can easily schedule a make-up at this link (http://bit.ly/LBMAKEUPS). Please keep in mind that make-ups may be scheduled at any location, as long as there is space available and you are currently enrolled. You can cancel your make-ups the same way you scheduled them. 

REFUNDS: Class fees are non-refundable. If your class needs to be canceled due to weather, teacher illness, or anything beyond our control, you will have a choice of participating in a make up class or a recorded music class. If there is a major illness surge, your indoor class may be moved online or to an outdoor location. If there is poor air quality, your outdoor class may be moved indoors. Thank you for your understanding.

SICK POLICY: We all want our little lovebugs to stay safe and healthy. Do not come to class if your child or their caretaker are sick. Here are some things to consider if you are trying to decide whether to attend class or stay home. If you or your child has a runny nose, fever within 24 hours, cough, vomiting or diarrhea, rash or any other illness, please stay home and rest instead of coming to class. We clean our instruments in between each class to keep germs to a minimum. We recommend that you wash or wipe hands when you arrive in class and wash hands when you leave. Hand washing with warm, soapy water has been proven to reduce the spread of germs... Don't forget your lovebug's hands. If you are showing any signs of sickness, but have been OK'd by your doctor as not contagious, please notify your teacher prior to the start of class so everyone will know that they are safe. Teachers have been instructed to request that anyone (child or adult) exhibiting the above symptoms to leave class and schedule a make up on our website.

AUTOPAY POLICY: By enrolling in our classes, you agree to our convenient monthly auto-payment system. Payments are processed on the 1st of each month for that month’s classes. If you enroll after the 1st, you will be charged a prorated amount for the remaining classes that month, with your first full monthly payment processed on the 1st of the following month. Your enrollment will continue automatically unless you choose to cancel. 

To make a class change request, please email hello@lovebugandme.com. Changes are subject to availability and will take effect only after you receive a written confirmation.

CHANGE OF TEACHER: We reserve the right to change the teacher assigned to your class, if necessary.

I HAVE CAREFULLY READ THIS RELEASE AND UNDERSTAND ITS CONTENTS. I AM THE PARENT OR GUARDIAN OF, OR OTHER PERSON AUTHORIZED TO GIVE THIS RELEASE AND AGREEMENT ON BEHALF OF THE CHILDREN PARTICIPATING IN THE ACTIVITIES.
